Long Term Care Facility is Hiring a Social Worker Near Middletown, NY
Skilled Nursing Facility, located in Orange County, New York, is currently looking for a team player to be our Social Worker.
The social workers functions will be to plan, develop, organize in accordance with social work standards of practice and current rules, regulations and guidelines. Collaborates with administrator, medical staff and other departments with Discharge Planning, MDS, care plan meetings with residents and family members and assessments of residents.
Employment Qualifications:
MSW required.
Knowledge of individual and group behavior; experience in working with community agencies and knowledge of services they provide.
Assessment, counseling and communication skills with the ability to relate to individuals of capacities in a helpful and respectful manner.
Discharge planning a must!
MUST HAVE LONG TERM CARE EXPERIENCE
